Zinc: Immune Function, Testosterone and Optimal Dosing

Zinc has a genuine immune evidence base, a real copper interaction and a narrow window between useful and counterproductive.

The Short Answer

Zinc is a cofactor for over three hundred enzymes and a structural component of thousands of transcription factors, with particular importance in immune cell function, wound healing, testosterone synthesis and skin integrity. Supplemental doses of 15 to 30 mg per day of elemental zinc are the usual range, and the evidence with the best support is for reducing the duration of the common cold when high-dose lozenges are started within a day of symptom onset. Prolonged intake above roughly 40 mg per day induces copper insufficiency, which is the main practical hazard and the reason zinc is one of the few supplements where more is clearly worse.

Why Zinc Matters

Zinc is the second most abundant trace metal in the body and its roles are structural as much as catalytic. Zinc finger motifs stabilise the DNA-binding domains of an enormous number of transcription factors, which is why zinc status affects gene expression broadly rather than through a single pathway.

Functionally, four areas matter most.

Immune function. Zinc is required for thymic function, T cell development and the activity of multiple immune cell types. Low status impairs both innate and adaptive responses, and the thymic involution seen in low zinc status resembles part of the immune aging phenotype.

Testosterone and reproduction. Zinc participates in testosterone synthesis and in sperm production. Low status is associated with reduced testosterone, and repletion in men with genuinely low zinc raises it; repletion in men with adequate zinc does not.

Skin and wound healing. Zinc is required for keratinocyte proliferation and collagen synthesis, which is the basis for its use in acne and wound contexts.

Antioxidant defence. Copper-zinc superoxide dismutase is a primary antioxidant enzyme, which is one reason the zinc-copper balance matters rather than zinc alone.

The Common Cold Evidence

This is the strongest zinc evidence and it is highly specific, which is why it is so often misapplied.

Meta-analyses of randomised trials find that zinc lozenges reduce the duration of common cold symptoms, with effect sizes in the range of a day or more, under specific conditions: high dose, typically 75 mg or more of elemental zinc per day in divided lozenges; started within twenty-four hours of symptom onset; and in a lozenge formulation that keeps zinc in contact with the throat rather than a swallowed capsule.

The proposed mechanism is local: zinc ions interfere with rhinovirus replication and with intercellular adhesion molecule binding in the nasopharynx. That mechanism explains why the formulation and the timing matter so much, and why a daily 15 mg capsule taken for general immune support is not the intervention that was tested.

Trial results also depend on the lozenge composition, since certain flavourings and binders such as citric acid chelate zinc and prevent ion release, which accounts for some negative trials.

Separately, zinc supplementation reduces the incidence and duration of diarrhoeal illness in children in populations where zinc status is commonly low, which is a well-established public health finding in a different context.

Testosterone: What the Evidence Supports

Zinc appears in testosterone products constantly, and the honest summary is narrower than the marketing.

In men with genuinely low zinc status, including those with restricted diets, malabsorption or heavy sweat losses, repletion raises testosterone measurably. In men with adequate status, supplementation does not raise testosterone in trials.

Exercise and heat are relevant because zinc is lost in sweat, and athletes training heavily in hot conditions can have meaningfully increased requirements. That is a real case for attention rather than a general one.

The practical implication: zinc is worth having adequate, and it is not an androgenic intervention. Anyone taking a zinc-magnesium-B6 product for testosterone should know that the trials supporting the concept were conducted largely in athletes with low baseline status.

Dose, Forms and the Copper Problem

  • 15 to 30 mg per day elemental. The general supplemental range, comfortably covering requirements alongside diet.
  • 40 mg per day. The commonly stated tolerable upper intake level from all sources. Chronic intake above this reliably induces copper insufficiency.
  • 75 mg or more per day, short term. The cold lozenge regimen, used for days rather than weeks.

Forms. Zinc picolinate, citrate, gluconate, acetate and bisglycinate are all reasonably absorbed. Zinc oxide is poorly absorbed and common in cheap products. As always, the elemental content is the number that matters, not the compound weight.

The copper interaction. Zinc induces intestinal metallothionein, which binds copper and prevents its absorption. Sustained high zinc intake therefore produces copper insufficiency, which presents with anaemia, low white cell counts and, if prolonged, neurological effects that may be irreversible. This is not theoretical: it is a documented consequence of long-term high-dose zinc use, including from denture adhesives and from enthusiastic supplementation.

The practical rule: stay at or below 40 mg per day for sustained use, or include copper at roughly a fifteen-to-one zinc-to-copper ratio if using higher doses for an extended period, and reassess rather than continuing indefinitely.

Absorption notes. Zinc competes with iron and calcium for absorption, so separating them helps. Phytates in grains and legumes bind zinc and reduce absorption substantially, which is why plant-based diets have higher effective requirements. Taking zinc on an empty stomach improves absorption and causes nausea in many people, so with a small amount of food is the usual compromise.

Who Is Most Likely to Be Short

  • People on plant-based diets, given phytate binding and lower zinc density in plant foods.
  • Older adults, with reduced intake and absorption.
  • Athletes with heavy sweat losses, particularly in heat.
  • People with malabsorption conditions, inflammatory bowel conditions or after bariatric surgery.
  • Heavy alcohol users, since alcohol increases urinary zinc losses.
  • People on long-term diuretics or certain other medications that increase excretion.

Testing is unsatisfying: serum zinc is the common test and is a poor reflection of body stores, falling during inflammation independently of status. There is no convenient accurate marker, which means dietary assessment and clinical context carry more weight than the number.

Practical Summary

Zinc rewards adequacy and punishes excess, which is unusual among supplements and worth respecting.

For general use: 15 to 25 mg per day of a well-absorbed form, with food, and not indefinitely without reassessment. For the cold indication: high-dose lozenges within twenty-four hours of onset, for the duration of symptoms only, checking that the lozenge does not contain citric acid. For anyone on a plant-based diet or training heavily in heat: a stronger case for routine supplementation at the lower end.

The review triggers are specific. Any sustained use above 40 mg per day is a reason to add copper or stop. A full blood count showing unexplained anaemia or low white cells in someone taking high-dose zinc should raise copper status as a question. And any new supplement or fortified product should be audited for the zinc it contributes, since totals accumulate invisibly.
