Selenium: Thyroid Support and Antioxidant Protection

Selenium has the narrowest window between inadequate and excessive of any common supplement, and geography decides where you start.

The Short Answer

Selenium is incorporated into selenoproteins including the glutathione peroxidases, thioredoxin reductases and the deiodinases that convert thyroid hormone T4 into active T3. Adult requirements are around 55 micrograms per day, typical supplemental doses are 100 to 200 micrograms, and the tolerable upper limit is 400 micrograms from all sources, which is one of the narrowest windows in nutrition. Status depends heavily on where food is grown, since soil selenium varies by orders of magnitude across regions. Supplementation benefits people with genuinely low status and has produced unfavourable metabolic signals in trials of people who already had enough.

Selenoproteins and What They Do

Selenium is unusual among trace elements: it is incorporated into proteins as the amino acid selenocysteine, encoded by a repurposed stop codon. Around twenty-five human selenoproteins are known, and their functions cluster in three areas.

Antioxidant defence. The glutathione peroxidases reduce hydrogen peroxide and lipid peroxides using glutathione, and thioredoxin reductases maintain the thioredoxin system. These are among the primary cellular antioxidant enzymes, and unlike dietary antioxidants they are enzymatic and regenerative.

Thyroid hormone metabolism. The iodothyronine deiodinases convert T4 to the more active T3 and inactivate hormone when needed. The thyroid gland has the highest selenium concentration of any tissue by weight, which is a strong hint about priority.

Immune and reproductive function. Selenoproteins participate in immune cell function and in sperm maturation, where selenoprotein P delivery to the testis is essential.

Geography Determines Status

Selenium content in food reflects soil selenium where the food was grown, and soil selenium varies enormously. Parts of China, New Zealand, Finland before fortification, and areas of Eastern Europe have low-selenium soils; parts of North America and Venezuela have high-selenium soils.

This has practical consequences that generic advice ignores. A person in a low-selenium region eating locally grown food may have genuinely low status; a person eating a couple of Brazil nuts a day, which can each contain 50 to 90 micrograms and sometimes far more, may be near the upper limit without realising it.

Brazil nuts deserve their own warning: their selenium content is extremely variable, and cases of selenium toxicity from habitual heavy consumption are documented. Two nuts a day is a reasonable and often-quoted approach to adequacy; a handful a day is not.

The Evidence, by Outcome

Thyroid autoimmunity

The best-supported use. Trials in people with autoimmune thyroiditis report reductions in thyroid peroxidase antibody concentrations with 200 micrograms per day of selenium, particularly selenomethionine. Whether that translates into clinical benefit, meaning better thyroid function or reduced progression, is less clear and meta-analyses are cautious. It is also studied in mild thyroid eye disease, where a trial reported improved quality of life and reduced progression at 200 micrograms per day for six months, which is a stronger clinical result.

Cancer prevention

The story that reversed. Early trials in low-selenium regions suggested reduced cancer incidence, prompting large prevention trials. A major trial of selenium and vitamin E in men with adequate baseline selenium found no cancer prevention benefit and reported an increased incidence of type 2 diabetes in the selenium arm, alongside an unexpected increase in prostate cancer in the vitamin E arm. The lesson was general rather than specific to selenium: supplementing a nutrient in people who already have enough can produce harm rather than benefit.

Glycaemic effects

Several analyses associate high selenium status with impaired glucose handling and higher type 2 diabetes incidence. The relationship appears U-shaped, with both low and high status unfavourable.

Mortality and immune function

Observational data associate low selenium status with higher all-cause mortality and impaired immune responses, with the associations concentrated at the low end of the distribution.

Dose, Forms and the Upper Limit

  • 55 micrograms per day. Adult requirement in most guidance.
  • 100 to 200 micrograms per day. Common supplemental range, and the dose used in thyroid trials.
  • 400 micrograms per day. Tolerable upper intake level from all sources including food. Not a target.
  • Above 800 micrograms per day chronically. Selenosis territory: hair loss, brittle nails, garlic breath odour, gastrointestinal upset and neurological effects.

Forms. Selenomethionine is well absorbed and incorporated into general protein, creating a body pool that buffers intake. Sodium selenite and selenate are inorganic, absorbed differently and used directly for selenoprotein synthesis. Selenium-enriched yeast contains a mixture and was used in several trials. For thyroid antibody trials, selenomethionine is the form most often used.

Testing. Plasma or serum selenium reflects recent intake; whole blood or erythrocyte selenium reflects longer-term status; glutathione peroxidase activity is a functional measure that plateaus once requirements are met, which makes it useful for identifying inadequacy but not for measuring excess.

Practical Guidance

Selenium is a nutrient to be adequate in rather than optimised upward, and the practical approach follows from that.

If you live in a low-selenium region or eat a restricted diet, 100 to 200 micrograms per day is reasonable, or two Brazil nuts most days as a food-first route. If you have autoimmune thyroiditis or mild thyroid eye disease, 200 micrograms per day of selenomethionine has trial support and is worth discussing with whoever manages your thyroid. If you already eat Brazil nuts regularly, a supplement on top is how people reach the upper limit without noticing.

Sustained supplementation above 200 micrograms per day has no benefit case in someone with adequate status and has a specific glycaemic signal against it. This is one of the few supplements where a member's most useful action may be to stop.
