Preparing for a Blood Draw: How Not to Ruin Your Own Panel

A hard session three days earlier, a supplement you forgot to mention or an afternoon appointment can change results enough to trigger the wrong decision.

The Short Answer

A well-chosen panel drawn under the wrong conditions produces misleading results at no saving, and the conditions cost nothing to control. Intense exercise in the preceding 72 hours raises liver enzymes, creatine kinase and hs-CRP while lowering fasting insulin. An afternoon draw halves a testosterone result. Biotin distorts thyroid assays. Each of these has caused unnecessary investigation, and each is avoidable with a few days of planning.

The Non-Negotiables

Fasting, where required. Eight to twelve hours, water only, for fasting glucose, fasting insulin and, where triglycerides specifically matter, the lipid panel. Coffee, even black, affects some results and is best avoided. A genuine fast means no food, and "just a little" invalidates it.

Morning timing, where required. Before 10am for testosterone, since diurnal variation is large enough to move a result across a clinical threshold and reference ranges were established on morning samples. Cortisol is even more timing-dependent. Iron studies also have a diurnal rhythm.

No unaccustomed or intense exercise for 48 to 72 hours. This is the most commonly overlooked condition and it affects several analytes: AST and ALT rise, creatine kinase can rise substantially, hs-CRP rises, and fasting insulin falls because acute exercise improves insulin sensitivity for up to 48 hours.

Not during or within a few weeks of acute illness. Illness raises hs-CRP and ferritin, lowers cholesterol, distorts thyroid results and lowers testosterone. A panel taken after a chest infection can look like a different person.

Cycle day recorded, where female reproductive hormones are included. Without it the result is not interpretable.

Biotin is the one worth acting on specifically, because the interference is well documented, the doses in hair and nail supplements are high enough to matter, and it has produced results mimicking hyperthyroidism that led to unnecessary intervention.

Creatine deserves a note in the other direction: it raises creatinine modestly through normal metabolism rather than through kidney impairment, so declaring it prevents an eGFR result being misread. Stopping it for a routine panel is unnecessary and, if you do, note that you did.

The 72 Hours Before

Alcohol. Avoid for at least 48 to 72 hours. It raises triglycerides substantially, raises GGT, affects liver enzymes and disrupts sleep, which affects several other results.

Exercise. Normal activity is fine; unaccustomed, prolonged or maximal effort is not. If you have just run a race or completed an unusually heavy session, postpone rather than draw.

Diet. Eat normally. A deliberately clean few days produces a panel that does not represent your usual state, which defeats the purpose. Very high-fat or very high-carbohydrate meals the previous evening affect triglycerides and glucose.

Sleep. A normal night. Restriction affects glucose, insulin, cortisol and testosterone measurably.

Hydration. Well hydrated but not overhydrated. Dehydration concentrates the sample, raising haemoglobin, haematocrit, albumin and urea; excessive water intake dilutes it.

Rapid weight loss or active fasting. Both raise uric acid, alter thyroid results and shift lipids. If you are mid-diet, either postpone or record it so the results are interpreted in context.

On the Day

Timing. Same time of day as any previous panel you intend to compare against.

Same laboratory, since between-assay differences can exceed real biological change, particularly for insulin, free testosterone, thyroid antibodies and Lp(a) where units differ.

Rest before the draw. Five minutes seated. Posture affects several analytes, and standing versus lying can shift results measurably.

Tourniquet time. Prolonged tourniquet application raises potassium and some other analytes. A competent phlebotomist manages this; it is worth mentioning if the draw is difficult.

Do not clench your fist repeatedly, which raises potassium.

Sample handling. For a posted kit, send it so it arrives promptly. Homocysteine rises in whole blood at room temperature, potassium shifts with storage, and glucose falls in unpreserved samples.

Blood pressure, if being measured, should be after five minutes seated, back supported, arm at heart level, not immediately after arriving or after caffeine.

The Common Ways a Panel Gets Ruined

An afternoon appointment for a testosterone panel. Produces a low result in a man with normal morning levels, and the most common error in this area.

A hard training session two days earlier. Raises liver enzymes, creatine kinase and hs-CRP, and lowers fasting insulin. Reported as multiple abnormalities.

A panel drawn a week after a cold. Raised hs-CRP and ferritin, lowered cholesterol, distorted thyroid. Looks like a systemic problem.

Female hormones without a cycle day. Uninterpretable, and the laboratory cannot supply the missing context.

Biotin not declared. Thyroid results mimicking hyperthyroidism.

Not fasting properly. A raised triglyceride and glucose result read as metabolic dysfunction.

Switching laboratories mid-sequence. An apparent change that is a method difference.

Testing during a deliberate diet or fast. Raised uric acid, shifted thyroid and lipids, read as new problems.

Each of these produces a finding, and findings produce actions. The point of preparation is not perfectionism, it is avoiding a decision made on an artefact.

A Simple Checklist

One week before: book a morning appointment. Note anything unusual coming up. If a hard event is scheduled, plan the draw before it or well after.

Three days before: stop alcohol. Avoid unaccustomed or maximal exercise. Stop high-dose biotin. Eat and sleep normally.

One day before: normal food, no unusually large or fatty meal, normal sleep, adequate water.

The night before: begin the fast if fasting analytes are included. Water only.

The morning: water, no coffee, no supplements unless instructed otherwise. Hold levothyroxine until after the draw. Note cycle day if relevant. Bring a list of everything you take.

At the draw: sit for five minutes first. Mention supplements, medications and anything relevant, including recent illness or exercise.

Afterwards: record the date, time, fasting state, laboratory, recent exercise, recent illness and cycle day alongside the results. That context is what makes the next comparison interpretable, and it is the step people skip.
