The NAD+ Stack: NMN, NR and Supporting Compounds
Most NAD+ stacks pay several times to fill one node while ignoring the consumption side entirely. A structured look at what a coherent NAD+ approach involves.
The Short Answer
A coherent NAD+ approach has two halves, and most commercial stacks address only one. The synthesis half means one precursor, not three: NR or NMN at trial-supported doses, or nicotinic acid at far lower cost with flushing as the trade-off. The consumption half means reducing what depletes the pool, principally inflammatory signalling through CD38 and DNA damage through PARP activity, alongside exercise, which raises NAMPT expression in muscle. Since several human trials have found no increase in muscle NAD+ despite large increases in blood NAD+, the consumption half is not an optional supplement to the synthesis half. It may be the more consequential of the two.
The One-Precursor Rule
NR, NMN, nicotinic acid and nicotinamide all terminate at the same node. Taking several simultaneously fills one slot at multiple prices, and there is no evidence that combining them produces more than an equivalent dose of one.
Worse, the combinations frequently work against each other. Nicotinamide, present in most B-complex and multivitamin products, inhibits sirtuins and PARPs at higher concentrations, which is the opposite of the intended effect of raising NAD+ to support those enzymes. A person taking NMN alongside a high-dose B-complex may be adding substrate and inhibitor at the same time.
The practical rule: choose one precursor deliberately, then audit every other product in the stack for hidden niacin and nicotinamide content, which is common and rarely noticed.
Choosing the Precursor
- NR at 250 to 1,000 mg per day. The deepest human safety record in this class, with reliable blood NAD+ elevation. Chemically unstable, so product quality and storage matter. Trials in healthy older adults have repeatedly failed to show functional benefit, which should temper expectations rather than eliminate the option.
- NMN at 250 to 900 mg per day. One step closer to NAD+ in the pathway, with modest functional results in metabolically compromised populations. Regulatory status in the United States is unresolved, which affects availability and quality control.
- Nicotinic acid at low doses. Raises NAD+ effectively at a fraction of the cost, with flushing as the obstacle. Gram-level doses are a pharmacological intervention requiring clinical monitoring of glucose handling and liver markers, and should not be self-directed.
- Nicotinamide. Cheap and effective at raising NAD+, with the sirtuin inhibition caveat at higher doses. It has its own separate and better-supported evidence base for skin outcomes.
Blood NAD+ appears to plateau rather than climb linearly with dose, so the upper end of these ranges buys less than the price difference implies.
The Consumption Side
NAD+ concentration is synthesis minus consumption, and the age-related decline is driven more by rising consumption than by falling synthesis. Three consumers dominate.
CD38. An enzyme on immune cells whose activity rises with inflammatory signalling and with age, and a major NAD+ sink. Anything that lowers chronic inflammatory load reduces this drain: visceral fat reduction, sleep restoration, addressing periodontal inflammation, smoking cessation.
PARPs. Consume NAD+ in response to DNA strand breaks. Ultraviolet exposure, tobacco smoke and chronic oxidative load all increase PARP activity and therefore NAD+ consumption. Sun protection is, mechanistically, an NAD+ intervention.
Sirtuins. Also consumers, though this is the intended use rather than a leak.
On the synthesis side beyond precursors, exercise increases NAMPT expression in skeletal muscle, which raises local salvage capacity in exactly the tissue where several precursor trials failed to show an increase. This is the single most defensible NAD+ intervention available and it costs nothing.
What About the Common Additions
Commercial NAD+ stacks typically bundle several compounds. Assessed individually:
- Pterostilbene or resveratrol. The pairing rationale is sirtuin-adjacent signalling. No controlled human trial has shown the combination outperforming the precursor alone. Pterostilbene has better bioavailability than resveratrol and less human outcome data.
- Apigenin. Included as a CD38 inhibitor, which is mechanistically the right target. Human evidence that dietary apigenin meaningfully inhibits CD38 in vivo is absent, and it is a plausible idea rather than a demonstrated one.
- Quercetin. Also proposed as a CD38 inhibitor, with the same evidence gap, plus a real interaction profile through CYP inhibition.
- TMG (trimethylglycine). Added on the reasoning that NAD+ metabolism consumes methyl groups when nicotinamide is methylated for excretion, potentially depleting methyl donors. The reasoning is coherent and the human evidence for clinically relevant methyl depletion at these doses is thin. Adequate folate, B12 and choline intake addresses the same concern more directly.
- CoQ10, PQQ, alpha-lipoic acid. Mitochondrial compounds bundled for narrative coherence rather than demonstrated synergy with NAD+ precursors.
Bundled products are convenient and make it harder to attribute effects or intolerance, and they usually mean paying for several unproven additions to access one studied compound.
A Structured Approach
Presented as a structure rather than a prescription, since none of this has been validated as a protocol.
First, the consumption side and the free interventions. Exercise for NAMPT expression, sleep for inflammatory load, sun protection for PARP activity, and addressing any identifiable chronic inflammatory source. Give this eight to twelve weeks on its own if starting from a low baseline.
Then one precursor, if still wanted. Single compound, trial-supported dose, morning, consistently, with every other product audited for hidden niacin content.
Set the window before starting. Blood NAD+ changes within one to two weeks; any functional change in trials took six to twelve weeks. Twelve weeks at a stable dose is the minimum fair test.
Define the endpoint. This is the hardest part and the most important. NAD+ status is not perceptible. Trainability, recovery between sessions, objective walk or workout data, and any relevant laboratory marker are the usable signals. Subjective energy tracks sleep, caffeine and glycaemic stability far more than it tracks NAD+.
Then decide. A twelve-week trial with no change on a defined measure is a result, and the correct response is removal rather than dose escalation.
Safety and What to Check First
Three checks belong before any of this.
Cancer history. NAD+ biology intersects with cell proliferation, and the consequences of sustained pharmacological NAD+ elevation in humans are unknown. Anyone with an active or recent cancer history should raise this with their oncology team rather than self-manage.
Prescription medication. Several of the compounds commonly bundled into NAD+ products, quercetin in particular, inhibit cytochrome P450 enzymes and affect drug concentrations. The precursors themselves have a cleaner profile than the additions.
Hidden niacin. Multivitamins, B-complexes, energy products and fortified foods contribute nicotinamide and nicotinic acid. Adding a precursor on top of an already substantial intake changes the arithmetic and may cross into sirtuin-inhibiting concentrations.
Long-term safety at these doses is unestablished, because the trials have run for weeks while the use pattern people adopt runs for years. That is not an alarm; it is the honest state of the evidence.
