Inflammation Biomarkers: What to Test and How to Read It
One inexpensive test does most of the work, several expensive ones add little, and the interpretive rule that matters most is not to measure during acute illness.
The Short Answer
High-sensitivity CRP is the most useful inflammatory marker available to a consumer, and it is cheap, standardised, widely available and prospectively associated with cardiovascular outcomes. Most of what is sold beyond it adds cost without adding decisions. The interpretive rules matter more than the panel breadth: CRP rises several-fold with any acute infection, so a measurement taken during or shortly after illness is uninterpretable, and a single value carries considerable biological variation.
High-Sensitivity CRP: The Core Test
C-reactive protein is an acute-phase protein produced by the liver largely in response to interleukin-6. The high-sensitivity assay measures the low concentrations relevant to chronic low-grade inflammation, as distinct from the standard assay used for acute illness.
The commonly cited cardiovascular risk stratification is below 1 mg/L for lower risk, 1 to 3 mg/L for intermediate, and above 3 mg/L for higher risk. Values above 10 mg/L generally indicate an acute process rather than chronic low-grade inflammation and warrant looking for a cause rather than interpreting as a risk marker.
Its strengths are practical: inexpensive, standardised across laboratories, responsive to intervention over months, and supported by a large prospective literature. Its main use in this Pillar is exactly the one most often skipped, which is measuring before and after a change so that the change can be evaluated.
Its limitations are real. Biological variation between measurements in the same person is substantial, so two readings are better than one. It rises with any infection, after intense exercise, in pregnancy and with oestrogen-containing contraception. Obesity raises it independently, which means it partly reports adiposity. And it is a downstream integrator rather than a mechanism-specific marker.

Neutrophil to lymphocyte ratio deserves a mention because it is free: any full blood count contains both numbers, and the ratio associates with outcomes in a broad range of contexts. It is nonspecific and it costs nothing to calculate.
Ferritin is the one where interpretation most commonly goes wrong. Because it rises with inflammation, a normal ferritin in an inflamed person can conceal low iron stores. Interpreting ferritin without a concurrent CRP is a recurring error.
The Rules for Measuring
Not during or within a few weeks of acute illness. CRP rises several-fold with infection and takes time to normalise. This is the single most common reason a measurement is uninterpretable.
Not within 48 to 72 hours of unusually hard exercise. Intense or unaccustomed exercise raises CRP transiently, and a marathon or an unfamiliar heavy session can produce a misleading result.
Note oestrogen exposure. Oestrogen-containing contraception and hormone therapy raise CRP without indicating an inflammatory problem.
Take two baseline readings where a decision depends on it, a few weeks apart, since within-person variation is substantial.
Same laboratory for comparisons. The assay is well standardised and consistency still removes a source of noise.
Interpret alongside adiposity. CRP partly reports fat mass, so a fall accompanying weight loss is expected rather than a separate finding.
Recheck at three months, not three weeks, after an intervention. Inflammatory markers respond over months.
When the Result Should Prompt a Clinician
Some inflammatory results are not optimisation findings, and continuing to adjust a protocol delays the thing that would help.
hs-CRP persistently above 10 mg/L without an obvious explanation. This suggests an active process rather than chronic low-grade inflammation.
Rising CRP with weight loss, night sweats, fever or fatigue. This combination warrants prompt assessment.
Elevated CRP with joint symptoms, particularly morning stiffness lasting more than an hour, which suggests an inflammatory rather than mechanical process.
Markedly elevated ESR with elevated CRP, which has specific clinical associations.
Any elevation with anaemia and raised platelets, a pattern of chronic inflammation with several causes worth identifying.
The point of listing these is that a wellness framing can absorb findings that belong in a clinical pathway. Chronic low-grade inflammation is a legitimate optimisation target, and a persistently and markedly elevated marker is a different category of finding.
A Sensible Panel and Schedule
Baseline: hs-CRP, full blood count with differential so the neutrophil to lymphocyte ratio can be derived, ferritin, HbA1c and a lipid panel with apolipoprotein B. This costs little and covers the inflammatory picture alongside the metabolic drivers that produce it.
Repeat hs-CRP at three and six months after an intervention, away from illness and hard exercise.
Annually thereafter as part of a general panel.
Skip: multi-cytokine panels, TNF-alpha, oxidised LDL and myeloperoxidase for routine use, and any panel whose report arrives with a supplement protocol attached.
Add, in specific contexts: fibrinogen in a cardiovascular workup, homocysteine where B vitamin status or vascular risk is a question, and whatever a clinician requests where an inflammatory condition is suspected.

Why Measurement Matters More Here
