Gut Testing: Microbiome Analysis and What It Can Tell You
Consumer microbiome reports describe composition accurately and interpret it beyond what the science supports. The clinically useful stool tests are different ones.
The Short Answer
Consumer microbiome tests can tell you with reasonable accuracy which organisms are in a stool sample. What they cannot do is tell you what that means for you, because there is no established healthy reference composition, individual variation is enormous, and the personalised recommendations generated from these reports are not validated. Meanwhile the stool tests with genuine clinical value, faecal calprotectin, faecal immunochemical testing, pathogen panels, are usually not the ones being sold for optimisation.
What Consumer Microbiome Tests Measure
16S ribosomal RNA sequencing. The most common and cheapest approach. It amplifies a conserved bacterial gene region and identifies organisms usually to genus level, sometimes species. It does not distinguish strains and does not directly assess function.
Shotgun metagenomic sequencing. Sequences all DNA present, giving better species-level resolution and information about which genes and metabolic pathways are present. More expensive and more informative.
Metabolomic add-ons. Some services measure short-chain fatty acids or other metabolites in stool. Faecal concentrations reflect the balance of production and absorption, so a low reading can mean less production or better uptake, which limits interpretation.
The measurement itself is generally sound. The interpretation is the problem, and it is worth understanding that the sequencing and the report are two separate products with very different levels of validation.
Why Interpretation Fails
There is no defined healthy microbiome. Composition varies enormously between healthy people, by geography, diet, age and ancestry. A composition common in one healthy population may be uncommon in another. Reports that grade your microbiome against an ideal are grading against a construct.
Stool is not the whole gut. A stool sample reflects the distal colon and luminal contents, not the mucosa-associated community or the small intestine, where much of the immune interaction happens.
Variation over time is large. Composition shifts with recent diet, so a sample taken after an unusual week describes that week.
Causality is mostly unestablished. Associations between taxa and conditions are abundant, and whether a given organism's abundance causes a state or reflects it is usually unknown.
The recommendations are not validated. Reports commonly generate dietary or supplement advice from composition. There is no trial evidence that following such advice improves outcomes relative to general dietary guidance, and different services can give different recommendations from the same sample.
Reproducibility between services is poor. The same sample sent to different providers can yield different profiles and different advice, which is itself an informative result.

Faecal calprotectin is the standout. It is inexpensive, well validated, and it separates inflammatory from functional bowel disease, which changes management entirely. Anyone with persistent gut symptoms is better served by this test than by a microbiome profile.
Coeliac serology belongs on any list of gut investigations because coeliac disease is common, frequently unrecognised, and requires being on a gluten-containing diet at the time of testing, which means going gluten-free before testing makes the result unreliable.
The Contested and the Unvalidated
SIBO breath testing. The concept is legitimate and the methodology is contested: thresholds, substrate choice and interpretation vary, and false positives and negatives are both common. Conclusions drawn from it drive prolonged antimicrobial protocols whose benefit is not well established.
Leaky gut panels. Intestinal permeability is real and measurable in research settings using sugar probe recovery. Consumer tests based on zonulin have limited validation, and zonulin assays have been questioned for what they actually measure.
Food sensitivity IgG panels. Not validated for identifying food intolerance. IgG antibodies to foods reflect exposure rather than reaction, and major allergy societies advise against using these tests for this purpose. They commonly generate long elimination lists that narrow the diet and reduce fibre and plant diversity, which works against gut health.
Comprehensive digestive stool analysis panels. Bundle validated and unvalidated components together, with interpretive narratives that exceed what the analytes support.
Organic acid tests for dysbiosis markers. Reflect diet, gut bacteria and renal handling simultaneously, which limits interpretation.
The IgG panel is the one that causes most harm, because the resulting eliminations reduce dietary variety, which is the intervention that actually helps.
When a Microbiome Test Is Worth Doing
For curiosity, knowingly. If you find it interesting and understand that the recommendations are not validated, that is a reasonable purchase. Read the composition and disregard the grading.
For research participation. Contributing to large cohorts advances the field and is a genuinely good reason.
For tracking a deliberate dietary change, using the same service, same sample handling, before and after. Diversity metrics are the most robust output and are more likely to be meaningful than any taxon-level finding.
Not for: explaining symptoms, which needs calprotectin, coeliac serology and clinical assessment; guiding supplementation, which is not validated; identifying food intolerances, which requires structured elimination and reintroduction; or determining what to eat, where general dietary guidance outperforms it.
The honest summary: a microbiome test is an interesting purchase and not a clinical one. What follows from it is almost always the same advice, more plant variety, more fibre, fermented foods, less alcohol, which you can act on without the test.
What Is Actually Worth Tracking
Symptoms, specifically. Stool consistency using a standard scale, frequency, bloating pattern and its relation to meals, and pain. A two-week symptom diary is more informative than any test for functional symptoms, and it is what a clinician will want.
Plant species count per week. The variable most strongly associated with diversity and the one you can change.
Fibre intake. Estimated rather than precise, and tracked as a trend.
hs-CRP. Since Pillar 6 feeds Pillar 3 through barrier integrity, an inflammatory marker is a reasonable indirect readout.
Ferritin and B12. Both can indicate absorption problems and both are cheap.
Response to specific changes. One dietary change at a time, held for three to four weeks, with symptoms recorded. This is how food triggers are actually identified, and it is what IgG panels claim to shortcut and cannot.
A person doing all of this and no microbiome testing would understand their gut better than someone doing the reverse.
